Are Health Apps the Cure for Anything That Ails You?

Health Policy Forum | By Rochelle Sharp*

With about 9,000 consumer health apps currently available in the iTunes store, it seems like almost all smart phone users can download their way to better health these days.
The store offers a mindboggling array of creative apps, including ones that calculate calories burned during exercise, create soundtracks to help people fall asleep, and display pictures that can elicit memories from Alzheimer’s patients. If the store doesn’t offer something for what ails you now, it probably will soon. The selections will proliferate within the next year, with an additional 4,000 consumer apps expected by next summer, industry experts say.

But all this innovation creates a bewildering set of problems. It’s hard to figure out what apps are available, let alone which work best. Health apps may have the potential to dramatically improve people’s lives, but those based on misleading or bogus information can cause serious harm.
“Apple isn’t testing apps for their scientific validity,” said Dan Cohen, a social worker who has reviewed apps for their effectiveness.

Given the stakes, it’s no surprise that the government is starting to regulate these smart phone applications. Just last month, the Federal Trade Commission brought its first cases against the makers of two health apps. Each claimed to cure acne with colored lights emitted from cell phones.
“Smart phones make our lives easier in countless ways, but unfortunately when it comes to curing acne, there’s no app for that,” the FTC chairman said, when announcing the crackdown. The agency cited the makers of AcneApp, which had sold about 11,600 downloads of its $1.99 app, and the developers of AcnePwner, which sold 3,300 downloads of its 99 cent app.

The Food and Drug Administration (FDA), meanwhile, proposed regulations this summer for apps that could be considered medical devices. The agency, which is seeking comments on its proposals until Oct. 19, may focus on apps that are accessories to established medical devices used by doctors, such as smart phone apps that can display X-rays.
It could also regulate apps that transform smart phones into medical devices by using sensors or other attachments. Already, the FDA has approved gadgets that turn smart phones into blood pressure-monitoring cuffs and pocket ultrasound machines.

Apps that connect to consumer devices, such as blood glucose meters, may be regulated, too, if the apps display or analyze the meters’ readings, the FDA says.
The majority of health apps will almost certainly not be considered medical devices and will escape government scrutiny. But some app developers are voluntarily going through the laborious FDA clearance process, in part, to convince the medical community that their products have real clinical value.

WellDoc, a Baltimore-based health care company, got FDA approval last year for its DiabetesManager, which provides automated diabetes coaching for patients. The app also was tested in a randomized clinical trial conducted by the University of Maryland’s medical school, which found that patients had a statistically significant improvement in their blood glucose levels after using the app for a one-year period.
Scientists have found flaws with other apps.

When a George Washington University professor conducted the first content analysis of behavior-modification apps, she discovered that few of the 47 smoking-cessation apps available in 2009 followed evidence-based health guidelines. Lorien Abroms, a public health professor, concluded that the apps had “serious weaknesses” because they did not link to quit lines or clinics or suggest ways for smokers to get social support from family and friends.
She argues that public health apps should be developed from evidence-based guidelines and should offer users the chance to receive text messages. Studies show that text messaging can help people change unhealthy behaviors.

Many public health organizations have developed useful apps. The American Heart Association has one to help people find walking paths, while the New York City Department of Health, in its effort to help reduce the spread of sexually transmitted diseases, lists places that give away free condoms.
Such health apps are already popular and may soon be ubiquitous – with 500 million consumers projected to be using them by 2015. Nine percent of smart phone owners surveyed last year said they use health apps, Pew Research Center study found. Younger people downloaded them most frequently, with 15 percent of smart phone owners between the ages of 18 and 29 using them. Black owners also had higher user rates, with 15 percent of those surveyed taking advantage of them, compared to 11 percent of Hispanics and 7 percent of whites.

The most popular types of consumer health apps focus on fitness and diet, according to a report released last month by MobiHealthNews, which tracks mobile health issues. About 16 percent of available apps in the iTunes store focus on cardio fitness, with some running apps claiming millions of users, MobiHealthNews says. Another 14 percent of apps are aimed at improving people’s diets, while another 11 percent deal with stress and relaxation.

“Consumers trying to keep up with the ever-changing app market may want to consult medical societies, like the American Cancer Society,” says Cohen, who evaluates apps. Alternatively, they could download Cohen’s own app, Healthful Apps, which highlights some of the most effective, user-friendly health apps available. Cohen said he spent more than a year perusing and testing apps before coming up with his list. He focuses on apps in eight categories, including relaxation, caregiving, memory improvement, and communication. Among his favorites: iTriage, an app designed by two emergency room physicians that helps patients understand their symptoms and provides information about the closest clinics and hospitals – including wait times at some emergency rooms.

Cohen says that he sometimes uses the Asleep app when he has insomnia, and also likes sleep apps that send different sound tones to each ear, called binaural tones. He is a fan of Panic Control, an app designed by a California psychiatrist that helps people during panic attacks, as well as apps that test people’s hearing and vision. He likes such testing apps, not because they can replace doctors, but because they can serve an important public health function of encouraging people to seek professional help when they need it.
For caregivers of the elderly, Cohen likes Elder 411, an app designed by a geriatric care manager filled with more than 500 pieces of advice, and Alzheimer’s Cards, an app filled with pictures that can spark memories, based on the book, “Simple Pleasures for Special Seniors.”

For an app that has completely transformed the lifestyle of people with disabilities, he picks Proloquo2Go, a text to speech app that allows people with autism, cerebral palsy, and various brain injuries to communicate more easily. Cohen also endorses Pain Care, which recently won the “Project HealthDesign” challenge sponsored by the Robert Wood Johnson Foundation and California HealthCare Foundation. The app lets patients track pain symptoms and share data with their doctors. It is designed to help people figure out what exacerbates their pain and what treatments are most effective.

It’s impossible to know how the next generation of consumer apps could change patients’ lives. But almost everyone agrees with one statement made by FDA deputy director William Maisel, when he spoke at the agency’s recent hearings on apps and said.
“These types of products will revolutionize the way we deliver health care.”

New job opportunities emerging in Big Pharma

Since 2001, 300,000 pharma employees have lost their jobs, primarily in R&D and sales. That's according to Clifford Mintz, the founder of BioInsights, which develops and offers bioscience education and training. Mintz spoke at a session on new job opportunities in biotech and pharma at the annual AAPS meeting in Washington, D.C. While the losses have been steep, they're balanced by emerging, in-demand careers in the industry.

The industry's struggles are well-known: Many companies are facing loss of exclusivity on their biggest sellers but have little in the pipeline to pick up the slack. Productivity is dropping as the cost of bringing a new drug to market soars. Government and payors want more effective drugs for less money. The list goes on.

Developers are looking to new markets and new technologies to address these issues. But how do these trends play out for the pharma job seeker? Many people, particularly Ph.D.s, may have to consider getting additional training if they want to land their dream job. "Companies used to be willing to just hire smart people. But with the economic downturn and global competition, companies can no longer afford to invest in people who have promise. They need to see proven skills," Mintz explained. With the right blend of skills and experience, however, there still some pharma jobs that are in demand.

Clinical Research and Regulatory Affairs

"Clinical research is the lifeblood of the industry," Mintz said. As developers expand in emerging markets, there's a particular demand for people to manage and organize overseas clinical trials. "There's a huge need for clinical research professionals worldwide," he said, noting that most Phase I and II trials are conducted outside of the U.S.

Another one of the industry's perennial needs is regulatory affairs professionals. "Regulatory affairs experience is a skill that all companies large and small would die to get their hands on," explained Mintz. The increasingly complex and uncertain world of FDA regulation--particularly when it comes to new technology and science--means that companies are always on the prowl for individuals with solid regulatory knowledge and ability to interact with the FDA.

 Biomanufacturing

The pharma industry's interest in biologics remains strong--just look at Sanofi's buyout of Genzyme, or Roche's purchase of Genentech. They're lured by disease-altering biologics that are less likely to face generic competition than traditional drugs. As a result, there's been increased demand for professionals who can navigate the complex world of biomanufacturing. Those with a background in upstream and downstream processes, large-scale protein purification, fermentation technology and bioengineering can make the transition to biomanufacturing.

Healthcare Information Technology

The rise of bioinformatics and genomics coupled with the push for electronic medical records has created jobs in healthcare information technology. Health informatics--the intersection of healthcare and IT--is ideal for people with expertise in genomics, bioinformatics or software that understand how to work with and manipulate large data sets and databases. The Obama administration has made EHRs a priority, and there's a need for software engineers and biologists who are comfortable working with medical information.

Medical Devices

"The medical devices industry has been experiencing explosive growth for the past decade," Mintz said. Regulatory hurdles in the medical device industry are much lower than they are for biologics or small molecules, making the industry a more stable alternative to biotech and pharma. The demand for devices, which address problems that can't be treated with medicine, will continue to grow as the population ages. Job seekers with strong backgrounds in bioinformatics, genomics, engineering and translational medicine are best suited to this field.

Medical Communications

Medical communications--which includes medical writing, editing, graphic design and science journalism--continues to boom. The demand for these jobs has risen because companies need a slew of communication materials to send to patients, physicians, researchers, investigators and the general public about their products and business.

Patent Law and Technology Transfer

Recent changes to U.S. patent laws have increased the demand for patent agents and patent attorneys in the life sciences field. Pharma's growing reliance on basic research from learning institutions means that there's a need for technology transfer experts. These experts manage the patent estate and intellectual property of universities and colleges that may engage in licensing deals with the industry. A law degree is a must to compete in this field.

10 largest U.S. patent losses over the next 12 months

In the drug business, exclusivity is everything. Once a branded drug gets generic competition, pricing power fades and sales slowly drain away, leaving drugmakers dependent on the next new thing. It's all very reasonable in theory, but when the drugs losing patent protection are the biggest in the business--and they're all falling off patent around the same time--even the most rational companies can get a bit panicky.

Welcome to the patent cliff, the most influential feature of today's pharma landscape. Here, so many drugs are falling off patent that the path forward has twisted, and the entire industry is looking to find its way. Some companies, such as Pfizer and Merck, have merged with major rivals. Others have snapped up acquisitions in ancillary businesses, such as generics and over-the-counter medicines, and expanded into countries they previously ignored. Still others have forged ahead, betting that they'd manage to develop enough new products to make up the difference.

Although a few major drugmakers have already suffered their biggest drops off said cliff, most are bracing for the fall. Here are the 10 biggest blockbusters that lose exclusive market rights in the U.S. over the next 12 months. They include some of the best-selling drugs ever, such as Pfizer's Lipitor and Bristol-Myers Squibb and Sanofi's Plavix. We ranked them not by U.S. sales volume, but by their weight in each company's U.S. revenue stream.

1. Forest Laboratories - Lexapro
2.
Takeda Pharmaceutical - Actos
3.
Bristol-Myers Squibb - Plavix
4. AstraZeneca - Seroquel
5. Eli Lilly - Zyprexa
6. Pfizer - Lipitor
7.
Merck - Singulair
8.
Novartis - Diovan
9.
Teva Pharmaceutical - Provigil
10. Abbott Laboratories TriCor

Τι λέει όμως γι' αυτό η τελευταία και πιο πρόσφατη μελέτη της Επιτροπής Ανταγωνισμού της Ευρωπαϊκής Ενωσης;

Pharma needs to leverage the internet, says Google

World News | October 21, 2011

Despite healthcare related topics being one of the top search categories online, pharma is not leveraging the potential of the World Wide Web, a Google expert has said. Speaking at yesterday’s PharmaTimes Digital meeting, Jens Monsees, industry head, consumer goods and healthcare at Google, said there were several areas where pharma was falling short in optimising its presence online. He noted that pharma content needed to be better and in context so as to distinguish between healthcare professionals and patients. “If the content is right then people will read it. If it is wrong then no one will be interested,” he said. He also added that pharma did not invest enough in its websites.

Likewise, he believed pharma was stuck in the mind-set of sales reps and push marketing, which was increasingly not working, when the industry should be considering pull marketing via the internet. People use search engines because they are proactively interested in obtaining information, so based on healthcare search trends, pharma can tailor the marketing message and pull people to the information they want, he said.

However, a consistent trend that was being observed was that pharma companies were becoming less relevant compared with healthcare queries in total online, Monsees said. He believed a greater focus on disease awareness by pharma could help reverse that trend. One trick pharma should take advantage of is filtering key words and specific search terms to understand the search behaviour of healthcare professionals and e-patients and target content accordingly. “Pharma companies are not leveraging even 5% of this potential,” Monsees said.

However, while Monsees emphasised the importance for pharma to rethink its use of the online space, he admitted that pharma was not considered a key client by Google because of pharma’s low spend. Monsees also said there was a lack of sufficient information about what doctors actually searched for online and suggested that perhaps collaboration was needed to source this information.

Meanwhile, in a panel discussion, David McCormick, digital lead at Roche, said the industry needed to be savvier when it came to contextualising content. “We haven’t changed the way we have our content. Are we packaging our content in a way that allows us to be social? If our customer is in a social environment we should be there with them. It’s about push and pull tactics together.”

However, Sandra Muzinich, former UK e-capabilities lead at Eli Lilly, said pharma needed to go back to basics and have a strategy. “Why do you want to go digital? That’s where we fall down in this industry. We are so eager to do the new thing we forget about why we want to do it and when we do it and fail we don’t get any more money to do it again.” She also added that pharma needs to ask more about what the customers want and actually analyse the data.
